- Leave a voicemail
- Submit an appointment request

- Upload identification or another document
- Receive general information from administrative personnel
A provider-patient relationship may be established only after applicable legal, clinical, administrative, consent, evaluation, and acceptance requirements have been completed and an authorized provider agrees to provide care.
WellnessMedX Patient Care Journey
WellnessMedX generally follows a provider-guided four-step process. The exact process may vary based on the requested service, individual needs, clinical appropriateness, provider recommendations, and applicable requirements.
Schedule an Introductory Call
Request an introductory call to discuss wellness goals, available services, general questions, and possible next steps.
Complete Labs or an Evaluation When Appropriate
Depending on the requested service, laboratory testing, health history, an evaluation, or additional information may be recommended or required.
Receive a Personalized Care Plan
When clinically appropriate, an authorized provider may develop personalized recommendations based on individual goals, history, evaluation findings, laboratory information, clinical judgment, and applicable standards.
Continue With Ongoing Support and Adjustments
Follow-up support, monitoring, repeat evaluation, repeat testing, or care-plan adjustments may be recommended when appropriate.
Introductory Call Requests Are Not Confirmed Appointments
Submitting the WellnessMedX Schedule Call form is a request for a WellnessMedX team member to contact you.
Submission of the form is not a confirmed appointment.
An introductory call or other appointment is considered scheduled only when:
- You complete a booking through an authorized WellnessMedX scheduling calendar; or
- A WellnessMedX team member creates and confirms the appointment.
After a call request is submitted, WellnessMedX may contact you using your selected or authorized communication method.
Response timing may vary based on business hours, staffing, request volume, holidays, technical availability, and the ability to reach you.
Eligibility and Provider Discretion
Access to the website, completion of an online form, payment for a preliminary service, or completion of laboratory testing does not guarantee eligibility for a particular treatment, program, prescription, procedure, medication, or ongoing service.
WellnessMedX providers retain appropriate clinical discretion and may determine that:

- A service is not clinically appropriate
- A service is not available in your location

- WellnessMedX is unable to accept or continue care
A provider may modify, decline, pause, or discontinue a recommendation or service when reasonably necessary for clinical, legal, safety, licensing, compliance, or professional reasons.
Appointments
You agree to provide complete and accurate information when requesting, scheduling, or attending an appointment.
You are responsible for:
- Reviewing appointment details
- Maintaining accurate contact information
- Attending scheduled appointments
- Following preparation instructions
- Providing requested information and documentation
- Informing WellnessMedX when information changes
- Requesting cancellation or rescheduling as early as possible
- Following any disclosed cancellation or rescheduling policy
Appointment availability is not guaranteed.
WellnessMedX may reschedule, cancel, modify, delay, or decline an appointment when reasonably necessary because of provider availability, incomplete information, clinical concerns, technical problems, emergencies, licensing limitations, safety concerns, severe weather, business interruption, or another appropriate reason.
Any cancellation, rescheduling, missed-appointment, deposit, payment, or refund policy disclosed during scheduling or purchase will apply to the applicable appointment.
Telehealth
WellnessMedX may offer telehealth services when clinically appropriate and legally permitted.
Telehealth availability may depend on:
- Your physical location at the time of service
- Provider licensing and authorization
- Provider availability
- Technology access and reliability
- The requested service
- Clinical appropriateness
- Applicable laws, rules, and professional requirements
Telehealth may have limitations compared with an in-person examination.
A provider may determine that an in-person visit, physical examination, local laboratory service, diagnostic testing, emergency evaluation, specialist consultation, or another form of care is more appropriate.
Access to the website or submission of a form does not guarantee eligibility for telehealth services.
Functional Laboratory Testing
WellnessMedX may coordinate comprehensive blood testing, functional laboratory testing, or related laboratory services when appropriate.

Failure to follow preparation or blood draw instructions may affect laboratory results and may require repeat testing.
Laboratory reference ranges, processing times, testing methods, result availability, and reporting formats may vary by laboratory.
Laboratory findings should be interpreted in the context of health history, symptoms, medications, supplements, evaluation findings, other testing, clinical judgment, and provider recommendations.
Do not make significant medication, hormone, treatment, or supplement changes solely because of a laboratory value without consulting an appropriately licensed healthcare provider.
Laboratory Fees and Payment
Certain WellnessMedX laboratory services may be offered on a self-pay basis.
The currently displayed price for the WellnessMedX Comprehensive Blood Tests laboratory panel is $350 self-pay, unless another amount is disclosed before purchase or service.
Payment timing may depend on whether the blood draw occurs:
- At the WellnessMedX Santa Clarita clinic; or
- Through an outside laboratory or authorized laboratory partner.
Additional costs may apply when disclosed, including costs associated with additional testing, repeat testing, outside laboratory services, consultation services, add-on services, or services not included in the selected laboratory panel.

Payments, Refunds, and Financial Responsibility
You agree to pay applicable fees disclosed before purchase or service.
Prices, fees, available services, membership terms, and promotional offers may change when permitted by applicable law.

WellnessMedX does not guarantee insurance coverage, reimbursement, health savings account eligibility, flexible spending account eligibility, or payment by another benefit program.

Telephone, Email, and Text Messages
When you provide contact information, request communication, submit a form, schedule an appointment, or otherwise authorize communication, WellnessMedX may contact you as authorized or legally permitted.
